Fill out an application explaining in detail what assistance you are requesting.

Send a written letter from your doctor stating that the medical condition is life-threatening.

Send a copy of your most recent tax record.

Provide copies of any forms that show your rejection from Medicaid.

Send copies of medical bills for which you are requesting assistance.

Use of WE CARE Funds

•Emergency funds are available, one time only for persons with a life-threatening illness who are in need of medication, or who have doctor bills up to a minimal amount. Emergency funds are available without completing an application by calling 734-289-3428 and speaking to a WE  CARE volunteer.
